Forms, guidance and workflow to collect contributions and informed consent

*All images, videos, audio clips, case studies and lived experiences which are shared with us by a child, young person or adult are referred to as ‘contributions’.

The following guidance, workflow and forms must be used by anyone who is collecting a contribution.

  • Barnardo’s Safeguarding in Communications Guidance; this document is to be used alongside the Contribution Collection Workflow and should be read before any contribution is collected.
  • Contribution Collection Workflow; this document is to be used alongside Barnardo’s Safeguarding in Communications Guidance and provides a step by step process to follow when identifying a suitable or collecting a contribution.
  • Conversation Recording Template; this should be used to record a summary of the conversation with child/ young person/ adult and then sent to relevant service to store on secure system.
  • Leave Behind Card; a copy of this must be given or sent to the person (or person’s guardian) who has shared their contribution.
  • Contribution Consent Form; consent form to be used for children/ young people/ family members/ volunteers etc. Refer to Barnardo’s Safeguarding in Communications Guidance.
  • Models Consent Form; consent form to be used for models.  
  • Consent forms in different languages; Albanian, Arabic, Bengali, Welsh, French, Kurdish, Latvian, Lithuanian, Mandarin, Polish, Portuguese, Romanian, Slovak, Somali, Turkish, Vietnamese.
  • Leave behind cards in different languages; Albanian, Arabic, Bengali, Welsh, French, Kurdish, Latvian, Lithuanian, Mandarin, Polish, Portuguese, Romanian, Slovak, Somali, Turkish, Vietnamese.
